A new passport can be issued:

  • If the current passport has expired. Once the new passport is issued, the previous passport is cancelled, but a valid visa remains.

Requirements

  1. Appropriate application form must be entirely filled in, signed by the applicant and submitted in person*
  2. 4 identical passport-sized snapshots [5x5cm]*
  3. Photocopies of relevant visa if current visa is still valid
  4. Photocopy of a Maltese Residency Card*
  5. Photocopy of Pakistani CNIC*

Modification in name

A change of name citing the following reasons can be issued through the appropriate application:

  • Marriage/Remarriage
  • Divorce
  • Death of spouse

Requirements

  1. Appropriate application form for change in name/surname*
  2. 4 identical passport-sized photos [5x5cm]*
  3. The current passport together with a copy of a valid visa*
  4. Original marriage certificate where applicable
  5. Divorce decree authenticated by court where applicable
  6. Copy of spouse’s death certificate where applicable
  7. Divorce decree of previous marriage/ Death certificate of previous spouse + Marriage certificate of new marriage where applicable
  8. Any additional booklets issued with the original passport together with the original passport.

In cases where the applicant would prefer a change/modification of name for reasons other than that mentioned above, the applicant is required to hand in the compulsory documents together with a sworn affidavit from a recognised authority.

Adding/Deleting spouse’s name

In such cases, applicants are required to apply for a new passport and append the relevant documents outlined below.

Requirements

  1. Entirely filled in and signed passport application form.*
  2. 4 identical passport-sized photos [5x5cm]*.
  3. Photocopies of valid visa/ Residence Permit.
  4. Original marriage certificate + photocopies of spouse’s passport.
  5. Divorce Decree authenticated by the court.
  6. Any additional booklets issued with the original passport together with the original passport.

In cases of remarriage, applicants are required to present marriage certificates of both marriages as well as the divorce decree of the first marriage/death certificate of previous spouse.

Change of address

In cases where the address of the home country needs to be changed, a new address in the form of utility bills, bank statements etc. must be presented in person.

Change of signature

For the signature on the new passport to be changed, the applicant must append the old signature along with the new one on the second page of the application.

  3. A passport with a current valid residence visa*

Change in appearance

If your appearance has changed significantly from your current passport, you can apply for a new one with a more recent photograph.

  3. Copies of a valid Visa where applicable
  4. Any additional booklets issued with the original passport together with the original passport

Emergency Documents

In case of a lost or damaged passport which needs to be replaced immediately, application must be brought in by person.

Requirements

  1. Entirely filled out and signed new passport application form*
  2. 4 identical passport sized photos [5x5cm]*
  3. A police report regarding the lost/stolen passport attested by MOFA, Malta*
  4. Photocopy of Pakistani ID Card*
  5. A photocopy of the lost/damaged passport
  6. Proof of a valid Visa/ Residence Permit*
  7. Any additional booklets issued with the original passport together with the original passport

If the applicant is not in possession of a valid visa, then only a short validity passport can be issued.

One-way travel Emergency Document

In cases of emergency passports, where no valid passport is available, only a document permitting outbound travel can be issued.

Requirements

  1. Filled in Emergency Certificate form*
  2. A letter from a sponsor*
  3. Affidavit signifying nationality*
  4. Proof of identity*
  5. 4 identical passport-sized snapshots [5x5cm]*

Issuing of passport to a minor (below 18 years)

Separate passports are issued children and infants. Minors are issued with a passport holding 5 years validity or valid up to 18 years of age, whichever is earlier if they possess the B-form issued from Pakistan.

Requirements

  1. Original birth certificate attested by MOFA, Malta*
  2. New passport form signed by parent*
  3. 4 identical passport-sized photos [5x5cm]*
  4. Photocopy of parents’ marriage certificate
  5. Photocopy of both parents’ Maltese Residency Cards*
  6. Photocopy of both parents’ Pakistani passports*
  7. Photocopy of both parents’ Pakistani CNICs*

If the child is unable to sign, a thumb impression is required in the signature box.
